During the 61st EASD Annual Meeting on September 18, IDF Europe held a symposium titled “Integration of care – the way forward”.
Building on last year’s symposium on personalisation of care, this year’s session explored the urgent need to advance integrated diabetes care across Europe. The programme showcased practical examples of integrated care models implemented in various European countries, highlighting successes and lessons learned.
Key experts engaged in discussions on ongoing efforts to address the fragmentation preventing the wider implementation of integrated care models across Europe, examining the main enablers and barriers to progress. The voice of people living with diabetes was at the heart of the conversation. They shared their experiences and expertise, explaining why integrated care is essential to achieving better health outcomes and leading long, fulfilling lives.
