SmartDiabetes – creating value through digitally enabled diabetes management models

SmartDiabetes is a project funded by the European Institute of Innovation and Technology (EIT) which aims to establish a comprehensive, value-based model for diabetes management, integrating digital therapy, data-driven tools and sustainable procurement practice to improve PwD’s health and reduce systemic costs.
By adopting a value-based healthcare model, digital therapy platforms such as “SocialDiabetes” – a key component of SmartDiabetes – might have the potential to transform care delivery and improve health outcomes for PwD.
IDF Europe’s role
As a key partner in SmartDiabetes, IDF Europe will ensure that the perspectives and needs of PwD are central to the project’s development and execution. IDF Europe will also support communication and dissemination activities, sharing the project’s progress and results with the broader community.
